NHTC has proudly provided services to adults with disabilities in the beautiful Black Hills of South Dakota. At NHTC, we strive to fulfill our vision of being the agency of choice, your choice! We provide services for everyday life, in the home, at work, volunteering, community, and much more.
Applicants must go directly through the Division of Developmental Disabilities to apply for services at NHTC. Here is a link to the Division website main page and “how to get services” which is at the bottom of the page: Division of Developmental Disabilities (sd.gov)
Additionally, here is a link to the state application guide for services.
Please note- There are exceptions to using the Division intake process – Please see below.
Due to the unique needs of individuals across the state of South Dakota, there are scenarios when a person must identify an opening at a Community Support Provider prior to final eligibility being established by DDD. This applies when individuals in the following scenarios are seeking services from a community support provider such as NHTC:
- Transitioning from the South Dakota Developmental Center or Human Services Center
- Transitioning from a nursing home or hospital
- School-aged children seeking CHOICES Child funding (i.e. receiving services from a CSP while below the age of 21)
- Moving from SD Family Support 360 to Northern Hills Training Center
Teams supporting individuals in one of the above scenarios will need to reach out directly to staff ([email protected]) to apply for services from Northern Hills Training Center.
If the applicant is accepted for admission by NHTC, then the Division of Developmental Disabilities will process and finalize the determination of whether the applicant meets eligibility requirements. Applicants from school age children who will be served PRIOR to turning 21 must have school district funding in place.
