The admissions process helps us reach that vision, and find you the right supports in all areas of life including vocational, residential, medical services, and day services. The following documents are required for admissions:
A completed application*
A social history report
A medical exam report from within the last year
Verification of the individual’s disability/disabilities
*The application can be filled out online (found under Admissions on the main page menu), or a hard copy can be requested from Cheryl Rohlf, Director of Residential Services, by calling 605-642-2785 ext 218 or emailing email@example.com. Cheryl will also be able to answer any questions you may have about the admissions process or any of the services we provide.
